Investigation Summary
At approximately 4:15 p.m. on June 1, 1990, Employee #1 was using a multi-rip saw to cut stickers to use as separators when stacking lumber. The saw began to plug up with sawdust and wood slivers, so Employee #1 pushed the STOP button to shut down the machine. He scooped one shovelful of sawdust of the floor and then decided to clean the saw blade area first. He lifted the guard with his left hand and reached into the blade area with his right hand. The blades had not stopped turning and his right thumb and three fingers were amputated. The blade area was appropriately guarded, but piles of sawdust prevented Employee #1 from seeing the blades.
