Investigation Summary
At approximately 1:00 p.m. on September 3, 1991, Employee #1, the woods boss, was spooling line on the skyline spool of an Edco yarder during normal logging operations. Employee #1 was attempting to get the line started and had stepped down between the frond spool and the frame so he could reach the line. He made eye contact with the operator to begin spooling the line and had just got it in the groove when his right foot was caught between the web of the spool and the frame. He hollered to the operator and had the spool backed up so he could remove his foot. Employee #1 was taken to Ride Out Hospital for treatment of the amputation of the third toe, tips of second and fourth toes (just past the nail), and the big toe (just past the nail). Only the big toe was reattached.
