Employee's toes amputated when caught in screw conveyor

At 3:50 p.m. on September 19, 1991, Employee #1 was vacuuming a rice truck dump hopper in preparation for the new rice season. The head dryerman had been watching him and, after helping him unplug the vacuum, decided to go to the control center to turn on the motors for the receiving equipment to ensure that they were functioning. As he was going through the start-up sequence, he routinely turned on the switch to the two augers (screw conveyors) in the dump bin, where Employee #1 was working. The augers had not been locked out. The dryerman immediately turned the augers off when he realized that Employee #1 was working in the hopper. By the time the dryerman left the control room, Employee #1 had climbed out of the hopper. He was taken to an urgent-care facility in Oroville, then transferred to Oroville Medical Center, where he was treated for an amputated left big toe and adjacent toe as well as bone involvement of the large bone behind the big toe. Reattachment of the toe was not possible.
