Employee's leg broken in same level fall

At approximately 10:30 p.m. on September 12, 1990, Employee #1 noticed a jam at the palletizer. He signaled the label machine operator, turned off his machine, and started to walk quickly over to the palletizer. En route, he tripped, fell, and struck his leg on the machine portion of the line before hitting the concrete floor. He suffered a compound fracture of his right femur and had to have a metal plate inserted in his leg. No tripping hazards or other safety and health violations were observed during the investigation. Employee #1 said he was not sure what caused him to trip.
