Employee's finger amputated during machine reassembly

At approximately 3:00 p.m. on July 24, 1990, Employee #1, age 16, was loading fruit onto a truck van. The loader consisted of a conveyor belt leading from the bottom of a fruit bin to the telescoping conveyor that carried the fruit inside the truck. Employee #1 became bored and decided to take off a hydraulic cylinder to see if the telescoping conveyor would operate with only one cylinder. As he was putting the unit back together, he stuck his right middle finger into the hole to line up the pin. He pushed the lever to adjust the hole, and it pinched his finger. He panicked and, instead of backing off the lever, he slammed it forward and amputated his finger. Employee #1 knew that in this operation, no adjustments were necessary. The accident was the result of his engaging in horseplay.
