Investigation Summary
At around 2:50 p.m. on April 16, 1991, Employee #1 was replacing a section of housing for the east white sugar elevator. To reach the upper bolts, he made a work platform using a 2 in. by 12 in. by 10 ft plank, which rested on a stepladder and a stairway. While he was tightening a bolt, the wrench slipped and Employee #1 lost his balance. He fell over the stairway railing and his head hit the bricks on the floor, about 6 ft below the work platform. Employee #1 was taken to John Muir Hospital for a skull fracture and back pain.
