Employee's fingers amputated in chain and sprocket drive

On July 9, 1993 at 7:45 a.m., Employee #1 amputated the middle and ring fingers (first joint) of the left hand on a chain and sprocket drive on the fire start wrapper machine in the fire log department. Apparently the guard, which is not adequately secured in place as required, fell off and exposed the employee to the in-running nip point of the chain and sprocket drive.
