Investigation Summary
At approximately 11:30 p.m. on October 7, 1992, Employee #1 was operating cereal packaging machine line #15. The boxes of cereal were kicking out because the top closure between the glassine liner and the box was out of alignment. Employee #1 looked under the vacuum on the machine to see which scale was collapsing the liner. When she reached over to grab and push up on the downspout of the filler machine, the tip of her left index finger became caught in the running equipment. Employee #1 suffered an amputation to her finger. Due to the prominent bone protruding through the amputation, a digital block was performed for which she was treated and released.
