Employee's right arm nearly amputated in separator

At approximately 5:30 p.m. on September 3, 1992, Employee #1, a swing shift cleanup person for Calva Products, Inc., was assigned by his supervisor to clean the separator (whirley sifter). Because of a lack of communication, a coworker was cleaning an auger that was part of the same system. When the coworker turned on the main power to the auger, the separator started and nearly amputated Employee #1's right arm. Employee #1 required hospitalization. The employer was cited for not effectively shutting down this operation for cleaning and for a not having an energy control program.
