Employee's hip injured in forklift tip-over

On October 23, 1993, Employee #1, a forklift operator, tipped over a Case forklift while driving down a levee access road. The forklift was not under positive control as required. Employee #1 was thrown from the forklift as it tipped over. His hip was injured when he was hit by the forklift's canopy.
