Employee injured when caught in garbage truck mechanism

Employee #1 was responsible for driving garbage trucks to the wash station at the rear of the yard, parking the trucks, cleaning them, and then moving them back. He had been specifically trained by his foreman to shut off the trucks before washing them and not to start them again until he drove them back to the parking area. At 4:15 p.m., Employee #1 was found with his right arm caught in the lifting mechanism that tipped the garbage containers. The truck engine was running, and the power take-off and the rev-up switches were both 'on' inside the cab. Employee #1 had been playing with the truck and its systems, in direct violation of the safety procedures that were taught for this job.
