ON NOVEMBER 14, 1990, EMPLOYEE #1 WAS STANDING ON A WOODEN HORSE (A METAL FRAME APPROXIMATELY 3 FEET IN HEIGHT AND 6 FEET IN LENGTH, SUPPORTED BY AN A-FRAME CONFIGURATION ON BOTH ENDS). EMPLOYEE #1 WAS IN THE PROCESS OF "HANGING" SHEETROCK TO THE CEILING OF THE SECOND FLOOR OF A RESIDENTIAL CONSTRUCTION, WHEN HE LOST HIS BALANCE AND FELL APPROXIMATELY 15 FEET TO THE CONCRETE FLOOR ON THE FIRST LEVEL. EMPLOYEE #1 SUFFERED A BROKEN RIGHT ARM AND RUPTURED LUNGS. THE LACK OF A GUARDRAIL CONTRIBUTED TO THE ACCIDENT.
