EMPLOYEE LACERATES INDEX FINGER ON UNGUARDED TABLE SAW BLADE

AT APPROXIMATELY 9:30 A.M. ON JULY 5, 1991, EMPLOYEE #1 WAS MAKING A STRAIGHT CUT WITH A POWERMATIC TABLE SAW, SERIAL #8368130, IN THE SHOP AREA. THE PIECE OF WOOD HE WAS CUTTING WAS APPROXIMATELY 3 INCHES BY 18 INCHES. EMPLOYEE #1 HAD PARTIALLY FINISHED THE CUT WHEN HE REACHED OVER THE BLADE AND HIS RIGHT HAND CONTACTED THE UNGUARDED SAW BLADE. EMPLOYEE #1 RECEIVED A LACERATION TO THE RIGHT INDEX FINGER. HE WAS TREATED AT THE HOSPITAL AND RELEASED ON THE SAME DAY. HE RETURNED TO THE HOSPITAL THE FOLLOWING WEDNESDAY AND HAD BONE REPLACEMENT SURGERY ON HIS FINGER. EMPLOYEE #1 WAS NOT HOSPITALIZED OVERNIGHT FOLLOWING THE SURGERY AND RETURNED TO WORK THE NEXT DAY.
