Employee injured in 12 ft fall through skylight

Employee #1 was cleaning up flour near a newly installed storage shed. While checking for deposits of flour around the fiberglass skylight, he slipped and fell approximately 12 ft through the skylight. Employee #1 struck the top of a metal bread cart, sustaining bruises and cuts to his face and chest. The roof was only slightly pitched (10:12), but the soles of his shoes had become smooth when coated with flour. This was the third time that Employee #1 had been sent to clean up flour near the storage shed. He had been working at the site part-time since October 15, 1990.
