Event: 10/19/1990
Employee injures back in fall from slipping ladder

At about 11:50 a.m. on October 19, 1990, Employee #1 was on an extension ladder passing plywood sheets to a coworker who was 4 to 5 ft above him on the roof of a residential building under construction. The unsecured ladder was positioned between two rafters, and was set at a steep angle. Employee #1 passed a sheet of plywood to the roof and was starting to descend when the ladder began to slip out. He jumped off to the side, striking a planked deck before landing on the floor. Employee #1 sustained serious back injuries, including two ruptured discs, and was taken by ambulance to Sierra Nevada Hospital in Grass Valley, CA.
