Employee's fingertip amputated in power saw

Employee #1, a sawmill conveyor operator, was working on a new conveyor that had been installed to cut up scrap lumber before it went into the mill. A fire had shut down the main mill and a chop saw had been put into the old conveyor, with the new conveyor running into it from the side. The saw was attached to an hydraulic ram. As the lumber was fed into the conveyor, it hit a metal plate that was hooked to a microswitch that stopped the conveyor. The saw then came down and cut the lumber into 18 in. lengths. The saw was guarded by a plywood box with a split conveyor skirt at each end. Some lumber became jammed and Employee #1 reached in to clear it when his hand was jerked into the box guard. The saw had came loose from the hydraulic ram and swung down so that it was within 3 in. of the switch instead of the normal 16 in. Employee #1's hand contacted the saw and the end of his index finger was amputated.
