Employee's fingertip amputated in pitching machine

At approximately 8:45 a.m. on March 15, 1991, Employee #1, age 17, was servicing a baseball pitching machine. He had just freed a ball stuck in the feeding mechanism when the slotted plate mechanism rotated and sheared off the end of his left little finger.
