Novice employee's hand crushed in roll machine

At approximately 1:15 p.m. on October 23, 1991, Employee #1 was asked by his supervisor/foreman to help a machine roll operator form circular reinforcing bands using a 4 ft roll machine. After completing the first band, the machine operator left the area to verify the fit. Employee #1 was left alone at the machine on his first day of employment and after only four hours on the job. He reached over the top of the moving rolls and had his fingers drawn into the machine. His left hand was crushed in the roll. Upon hearing Employee #1 scream, the machine operator returned and was able to stop the machine, open the roll, and release his hand. Employee #1 was transported to the hospital, where the first, third, and fourth fingers were amputated at the second joints.
