Investigation Summary

At approximately 9:30 p.m. on October 30, 1990, Employee #1, who had worked various jobs at the Georgia Pacific sawmill for 1 year, was in her third day of training as a trimmer operator. One of her duties as a trainee was to keep the conveyor clear. Someone noticed that conveyor #1523 was jammed. Employee #1 found the jam just outside the building, at the end of the conveyor. She used a pole pick to unjam the stuck board, then reached into the conveyor with her right hand to retrieve it. As she lifted it out, another board, which she did not see, struck her finger and amputated the tip. She was transported to Mendocino Coast Hospital and released on October 31, 1990. Employee #1 had received the company's lockout training in February, 1990.
