Investigation Summary

Investigation Nr: 170170054
Event: 08/10/1990
Employee bruised when barrel washer falls off forklift

Employee #1 had been hired in June 1990 to work on the bottle and package line in a brewery. On August 8,1990, a coworker asked him to help with re-setting an old barrel washer machine onto the new concrete pieces in the outside beer garden. The barrel washer machine was lifted over the wooden fence with a forklift. As it was being lowered, Employee #1 pulled on the barrel washer to align it with the piers. The barrel, which was not secured to the forklift, slipped off the forks and fell onto Employee #1, bruising his left knee. He was taken by ambulance to Isaiah Hospital, x-rayed and released. The employer was cited.
