Employee injured in fall when ladder slips out

Employee #1, of Walton Engineering, was climbing an extension ladder to the top of a 13 ft high gas station canopy. As he was about to climb onto the canopy, the ladder slipped out from under him. He fell onto concrete and sustained injuries that required hospitalization. The ladder was not secured to prevent it from being displaced. The employer was cited for a violation of T8CCR 1675(i). The employer also violated the company's own safety rules.
