Investigation Summary
Employee #1 was cutting strips from a 4 ft by 8 ft sheet of plywood when he was struck in the eye by a particle flying off the saw. The saw guard was connected to the left side of the table and only allowed 2 ft of clearance. Rather than switching the fence to the left side, the guard was removed. Employee #1 said he was wearing goggles, but they fogged up so he removed them.
