Employee injured in fall off ladder and canopy roof

On October 17, 1990, Employee #1, a 20-year-old carpenter, was assigned to hang plywood on the exterior canopy roof area of a multiple-cinema theater. He was working from an 8 ft ladder that was resting on the canopy roof. While he was applying plywood to the wall, the ladder slipped and fell to the canopy roof, causing Employee #1 to fall 23 ft from the canopy roof to the ground. He sustained a fracture of the fourth lumbar vertebra. The exact cause of the accident and possible accident-related violations cannot be determined, as the employee's current location is unknown.
