Investigation Summary
At approximately 2:15 p.m. on March 15, 1991, during the repair of a failed pump coil, the Vermiculite mixture that was in the attached hose hardened. When the pump was started, the hardened Vermiculite obstructed the line, and pressure built up until the 2 to 3 in. diameter hose blew off the threaded connector. It struck Employee #1's leg, fracturing it. He was taken by ambulance to Alta Bates Hospital and hospitalized for 3 days for surgery and traction.
