Employee's finger amputated in bottle maker

On January 27, 1991, Employee #1, of Anchor Glass Containers, was operating a bottle making machine. As he was making some adjustments, the distal portion of his right finger was amputated. The machine was not locked out prior to Employee #1 starting work. The employer maintains a documented training and discipline program, including compliance with lock-out policy. Employee #1, who had 17 years of experience, admitted that he violated company safety policy. This accident was determined to have resulted from an isolated, independent employee act.
