Event: 02/21/1991

Employee injures back in same level fall on roadway

At approximately 5:30 a.m. on February 21, 1991, Employee #1 was carrying her cash tray to her assigned tollbooth located at the opposite side of a construction zone. While walking around a steel stack that was blocking her path, Employee #1 fell to roadway and injured her back. She was immediately transported to St. Rose Hospital for evaluation and treatment, and was then sent home. Employee #1 continued to receive care on an outpatient basis and had not returned to work as of April 1, 1991. Causal factors involved in this accident include a blocked aisle and poor illumination.
