Employee injured in fall when ladder slides out

At 10:30 a.m. on January 21, 1991, Employee #1, a senior mechanic and leadsman, was helping another mechanic install the cover on belt conveyor #16B at a construction site. The coworker climbed a metal extension ladder and installed three bolts on the west side of the elevated conveyor. The ladder was then leaned against the east side of the conveyor and the coworker went inside the covered conveyor while Employee #1 ascended the ladder to align and secure the cover piece. The ladder was on compacted gravel; its top was leaning against a vertical conveyor member, with the ladder rails on each side of it. The ladder began to slip out from under Employee #1 and he grabbed the conveyor and held on for a moment as the ladder slid out from under him. Employee #1 dropped about 10 ft, landed on his feet, and then rolled onto the ground.
