EMPLOYEE INJURED WHEN A WORN RETAINING PIN DISCHARGES

AT 2:00 P.M. ON DECEMBER 6, 1990, EMPLOYEE #1 WAS ATTEMPTING TO REMOVE A WORN RETAINING DOWEL/PIN FROM THE HINGE BEARING ON THE UNDERCARRIAGE SWING FRAME OF A D7 CATERPILLAR DOZER. EMPLOYEE #1 WAS SITTING APPROXIMATELY 18 TO 22 INCHES FROM THE BEARING SEAT AND WAS APPLYING HEAT WITH A OXYGEN/ACETYLENE TORCH TO THE CAST IRON SURFACES AROUND THE DOWEL. HE WAS WATCHING THE DIAMETER OF THE DOWEL TO SEE IF THE OPENING BETWEEN THE DOWEL AND HEATED SURFACES WOULD OPEN UP. THE DOWEL, APPROXIMATELY 9/16 OF AN INCH IN DIAMETER BY 3/16 OF AN INCH LONG, SUDDENLY DISCHARGED FROM ITS HOLE AND STRUCK EMPLOYEE #1 IN THE RIGHT SIDE OF THE NECK. THE DOWEL HAD ENOUGH VELOCITY TO PENETRATE HIS NECK APPROXIMATELY 2 1/2 TO 3 INCHES. EMPLOYEE #1 WAS TRANSPORTED TO EDEN HOSPITAL AND ADMITTED AFTER SURGERY WAS PERFORMED FOR REMOVAL OF THE DOWEL. HE WAS RELEASED FROM THE HOSPITAL ON DECEMBER 9, 1990. EMPLOYEE #1 STATED THAT RESULTING NERVE DAMAGE IMPAIRS HIS ABILITY TO FOCUS HIS RIGHT EYE AND CONTROL OTHER MUSCLES ON THE RIGHT SIDE OF HIS HEAD.
