Employee injured when face struck by sledgehammer

At approximately 12:00 noon on August 6, 1990, Employee #1, a maintenance mechanic on a three-man maintenance crew, was completing the reassembly of a 500-ton #8 holding marine assembly system. The injection cylinders had been removed and repaired to correct a leak. Employee #1 and the two coworkers removed the massive metal yoke supporting the cylinder shafts. While the crew was reassembling the yoke, they had difficulty sliding it back onto the cylinder shafts. They were using a sledgehammer when Employee #1 was struck in the face by the hammer. He sustained injuries that required hospitalization.
