Investigation Summary
At approximately 11:00 a.m. on July 28, 1990, Employee #1, of Bayside Construction, was single nailing 4 ft by 8 ft plywood panels at a height of approximately 19 ft on the roof of a multiple two-story residence. He was moving laterally north to south on a 5:12 sloped roof when he said he stepped on a piece of scrap lumber and then fell to the ground. He suffered a fractured left tibia and a sprained left knee. Two general citations were issued for the violation of T8CCR 1513(a) and 1670(a).
