Employee pinned between forklift and metal frame

At approximately 10:00 a.m. on August 7, 1990, Employee #1 was conducting a weekly inventory when he bent down to read and record the stock number from a roll of carpet on a bottom rack. A material handler who was operating a Clark Model EC 500 forklift was preparing to retrieve a full roll of carpet from a rack directly across the aisle. The forklift had an attachment that gave it an overall length of approximately 19 1/2 ft; the aisle was 33 ft wide. Employee #1 was slightly pinned between the forklift and the rack's metal frame. He sustained multiple contusions to both sides of his chest and both arms and was hospitalized overnight for observation. The driver said he looked to his rear before moving but did not see anyone; Employee #1 says that he did not notice the forklift until he was pinned to the metal frame.
