Employee killed in fall from pole

On the morning of April 24, 1992, Employee #1, a utility/groundman for PG & E Engineering and Construction Department, Electric Transmission & Distribution Division, and an extended crew were installing a new 480 V distribution line. At about 8:30 a.m., Employee #1 was directed by the on-site supervisor to climb a pole about 200 ft from the double dead-end pole where the supervisor was working, and lay out cables (conductors) on the insulators in preparation for tensioning. Shortly after Employee #1 was seen putting on his belt and climbing spurs, the groundman working with the supervisor saw Employee #1 lying in the street. He had fallen. The supervisor descended from his work location, then directed his groundman to call EMS. A coworker administered first aid to Employee #1, but he died.
