Investigation Summary
Employee #1, of Owens-Brockway Glass Containers, Inc., had been temporarily assigned to operate case packers, but had received no training in the safe operation of these particular units nor in lockout procedures. After approximately two weeks on one unit, Employee #1 was switched to a similar one that had just been set up to containerize a different size bottle. Jams were frequent during the first few days, which was usual for a new setup. On December 11, 1991, a packing carton failed to reach the carton elevator prior to the bottles being released. One bottle jammed under the elevator. Employee #1 shut down the power to the unit and hammered at the jammed bottle with a second bottle until the jammed bottle shattered. With the jammed bottle no longer an obstacle, the elevator rapidly descended. Employee #1's right hand became caught between the bottom of the elevator and the frame. Employee #1 was taken by regional ambulance to Merritt Hospital. Her crushed right index finger was amputated to below the second joint. Serious accident-related violations of T8CCR 3202(a) and 3314(a) were cited.
