Investigation Summary
Employee #1 was replacing a bundling machine at the end of a newspaper mailing machine, which wraps bundles of paper with plastic cord. A coworker plugged the new unit in while Employee #1 was attempting to position the unit with his arm inside the area of the machine in which shear-like parts descend to apply the plastic cord. Employee #1 suffered minor arm bruises, which did not result in lost work time other than the day of the accident. The accident was caused by lack of communication between the two workers, and failure of Employee #1 to anticipate that the actions of his coworker could create a hazard. No accident-related violations were issued.
