Investigation Summary
At approximately 7:05 a.m. on October 19, 1992, while checking the glue tank on a taper from a two step stool, Employee #1 lost his balance and fell toward the taper machine. He braced himself against a box guide, which collapsed inward. The employee's hand went into the rollers, severely bruising and lacerating his fingers. Employee #1 required hospitalization. A general citation was issued for the violation of T8CCR 4002(a), unguarded component parts of taper machine.
