Investigation Summary
At approximately 11:00 a.m. on November 12, 1992, Employee #1, a trainee of Berkeley Tool and Forge, Inc., with 4 1/2 months experience, was adjusting a Toledo trim press, model #8000. The employee said that he was instructed by a coworker to insert a bar at the rear of the machine, which was to be struck with a sledge hammer. While standing on a mobile conveyor, Employee #1 lost his balance and grabbed the flywheel assembly with his gloved left hand. The hand was pulled into the gears, amputating his thumb, index, and middle fingers. The press had been turned off but was jogged into the desired adjustment. A serious accident-related violation of T8CCR 3203(a)(7)(b) was issued, along with general violations of T8CCR 3384(b) and 4184(b).
