Employee's finger partially amputated by pulley and belt

Employee #1, a maintenance mechanic for Anchor Glass Container Corp., accompanied a coworker, also a maintenance mechanic, to the top of a silo to help him adjust the belt tension on an elevator drive. The ground level operator saw the coworker signal and thought he wanted him to start up the system. The operator had not seen Employee #1 go up and was unaware that the coworker was signaling to Employee #1. The coworker wanted Employee #1 to signal the operator. As Employee #1 leaned over to signal the operator, he placed his hand on the drive belt for balance. The operator started the system up and Employee #1's left little finger was caught between the pulley and belt and amputated at the first joint. Employee #1 received medical treatment and returned to work without lost time. The employer's energy-control procedure did not include specific rules and techniques for servicing the equipment.
