Event: 11/02/1992
Employee killed when run over by crane

At approximately 1:12 p.m. on November 2, 1992, Employee #1, lead supervisory clerk for Stevedoring Services of America, was working near a transtainer, a motorized crane with four wheels used for lifting and moving large containers. He had just taken a bill of lading from a truck driver and was checking numbers on the containers. At the same time, the transtainer's operator was looking down through the crane cab's bottom window at the crane's alignment over the container stack and he noticed that he was too close to the truck parked next to the stack. The transtainer operator decided to move his vehicle. Employee #1, with his back to the approaching transtainer, stepped back from the truck directly into the path of the transtainer's right rear land-side wheel. He was knocked down, and his foot was caught under the wheel. Employee #1 was run over and fatally crushed. If proper guarding had been provided around the transtainer crane wheel, his death might have been prevented. A serious accident-related violation of 8CCR 4906(c) was cited.
