Employee burned in LPG explosion

At approximately 7:10 p.m. on June 16, 1993, an LPG explosion occurred at Granny Goose Food, Inc. Employee #1 was burned. The employee had worked as a warehouseman for about 36 years and had worked in the company's receiving department for about 5 years. He had worked as a full-time forklift operator for 3 years. His training as a lift truck operator consisted of watching training videos about two years ago. The training did not include practical exercises. The employer did not provide refresher training on an annual basis.
