Employee's leg is struck by falling swing frames

Employee #1 was picking up swing frames for a bulldozer with a forklift truck. The forks were elevated approximately 18 inches. Employee #1 was also picking up pins to carry with the load. He was looking away from the forks when one swing frame fell off and pinned his left leg. The second swing frame then fell off and fractured his left ankle. Each swing frame weighed approximately 1,500 pounds. Employee #1 was an experienced forklift operator, but he had not received training from the employer. A violation of 8CCR 3664(a) was determined.
