Employee's forearm fractured when caught in conveyor

Employee #1 was investigating why the sludge belt conveyor was slipping. He apparently suspected that the cause of the problem was a chemical compound that was added during the treatment of waste water Employee #1 was reaching into the head pulley to feel the surface when his right arm became caught in the nip point between the belt conveyor and the head pulley drum. He suffered fractures of his lower right arm, requiring surgery and the use of metal inserts.
