Employee's eye injured by flying metal piece

Employee #1 was using the pointed end of a hammer to knock out a locking pin that joined shaft sections of a coring machine. A piece of metal broke off from either the coring machine or the hammer and punctured his left eye, damaging the retina. Employee #1 was not wearing eye protection.
