Employee injured in same level fall

At approximately 11:30 a.m. on February 28, 1991, Employee #1 was taking boxed spare parts from a cart and shelving them in a store room. The aisle to the shelves was partially obstructed by a stored bandsaw. The gap between part of the bandsaw and the shelf unit was only 6 in. As Employee #1 squeezed through that gap, his foot became entangled in a loose strap hanging from the machine. He tripped and fell, striking the side of his head on the floor. Employee #1 suffered a cut left eye from the impact of his eyeglasses, but his hard hat protected him from other head injuries.
