Investigation Summary
At 2:30 p.m. on August 21, 1990, at the United Airlines maintenance operations center, a powder containing aluminum oxide used for coating turbine blades was being mixed in the new focus repair cell. Approximately 3,000 pounds of powder had been transferred to the #1 bin. Employee #1 had just left the powder mixing room and was standing outside the door. Employee #2 and three coworkers were also outside, standing near the control panel, when an explosion occurred in the mixing room. Employee #1 was struck by the powder room door and thrown against the building. He suffered broken ribs, a broken pelvis, a concussion, and a bruised lung and shoulder. Employee #2 was thrown back 8 to 10 ft. The employee emergency action plan was implemented and the building evacuated.
