Employee injured in fall from ladder

Employee #1, a welder, had welded extension tubes to a metal ladder. He was testing the ladder himself, without supervision, by placing it against the wall of the maintenance building while a coworker handed him the extension piece. The ladder he was standing on slipped and he fell sideways to the pavement. He suffered a broken pelvis and dislocated two fingers on his left hand. He was taken to San Jose Hospital for treatment.
