Investigation Summary
Employee #1 entered a walk-in refrigerator to get lettuce from a large plastic drum. The overhead light did not go on when she tried the light switch, although she noticed cheese and water on the floor. Employee #1 slipped on the wet floor and struck her head on a wire meat rack and then again on the floor. She was hospitalized with head injuries.
