Employee's fingers amputated in unguarded cutting machine

On October 4, 1991, Employee #1, of Fresh Foods of California, Inc., was operating an Urschel OV vegetable cutting machine. The cutting blades, which rotated in a circular motion, stopped moving because excess lettuce had become jammed in them. The employee put his right hand in the cutting blade area to clear the jam. Once the lettuce had been cleared, the blades began to circulate, amputating four fingers on the the employee's right hand. The guard for the cutting blades was not on the machine. Employee #1 was not trained in the safe operation of the machine or in the hazards unique to it.
