Investigation Summary
Employees #1 and #2, utility workers, were washing down the ash hoppers for a boiler. In the past, cold water had been used. This time, for the first time, Employee #2 was using hot water (75 degrees F) at a higher pressure. While moving the nozzle, he closed the valve. The hose fitting separated at the nozzle and the hose whipped around, spraying hot water. Employee #1, who had been working on the other side using cold water, happened to come around to the side where Employee #2 was working and was sprayed. She sustained burns on 30 percent of her body. Employee #2 was also struck by the scalding water but was not badly injured.
