Employees killed when crushed beneath skip hoist

At approximately 2:55 a.m. on May 16, 1990, Employees #1 and #2 were operating the wheelabrator in the foundry. They were evidently clearing a bad casting jam. They were between the wheelabrator and the skip hoist and shaker conveyor. Employee #1, in attempting to activate the shaker conveyor, hit the control for the skip hoist, which lifted and came forward, pinning the employees beneath it. The load of casting in the skip hoist weighed approximately 5490 lb. Both employees suffered massive head injuries. Employee #1 was dead at the scene and Employee #2 died approximately 15 hours later in the hospital.
