Two employees hit with debris from malfunctioning machine

Employee #2 was setting up equipment to dress a grinding stone in preparation for a machining operation. The 2 3/4 inch pulley was installed on the motor and the 7/8 inch pulley was installed on the spindle, the reverse of proper installation. With the motor turning at 10,000 rpm, this incorrect setup caused the stone to turn at approximately 30,000 rpm. Consequently, the grinding wheel blew up. A guard was not installed on the grinding stone and Employees #1 and #2 were hit with shrapnel.
